Worker’s Compensation Procedures
Employees must report any work-related injury/illness to their supervisor immediately.
If medical treatment is necessary, the employee should be transported to an Occupational Health Center or emergency room. (Hospital emergency rooms should only be utilized if the employee has a life-threatening injury or if there is not an Occupational Heath Center in the area).
At the treating facility, the employee needs to report the injury/illness as work related. Employees should not provide the facility with their group health insurance card. Employer information should be reported as WorkSmart Systems, 9957 Crosspoint Boulevard, Indianapolis, IN 46256, (317) 585-7870.
Supervisor should report the injury to WorkSmart Systems at (317) 585-7870. It is important to report the injury as soon as possible. The treatment facility will call WorkSmart for authorization and I will not approve treatment under worker’s compensation until I have verified that it is a work-related injury.
The telephone report should be followed-up with a written accident report. This needs to be completed within 24 hours of the incident and faxed to my attention at WorkSmart (317) 585-7880.
Employer should maintain contact with WorkSmart Systems concerning the employee’s condition, type of treatment, job restrictions, etc.
